MEDICAL RECORDS

Arizona Law (A.R.S. 15-871) requires
that documentary proof of immunization be on file for each
child enrolled in school.
Documented proof includes a record
which contains:

Child’s name
-
Birth date
-
Type of vaccine received
-
Month & Year of each immunization
(except for Measles, Mumps, & Rubella (MMR) for which
month, day and year is required.

Name of physician or agency
administering the vaccine
Exemptions from
immunizations are permitted for personal reasons (inclusive
of religious and philosophical reasons) with a parent’s or
guardian’s signature, and for medical reasons with the
signature of a parent or guardian and the signature of a
physician or other designated health professional.
Exempt students will not be
allowed to attend school during periods of disease outbreak
as determined by the local health department.
Title One Registration

Student’s registration for enrollment
cannot be completed until parents complete a Title One
registration. Title One provides qualified students the
opportunity to receive free or reduced meals at the school.
The Round Valley School District receives additional
benefits for many programs when the Title One information is
accurately reported. Some students may not qualify for
Title One services and others may choose not to participate
even though they are eligible. This new requirement will
benefit your student in many ways and your cooperation will
be appreciated
Permanent records are kept for every student entering Round
Valley Intermediate School. The record contains statistics about
the person and his/her academic achievements, standard test
scores and attendance.
The person enrolling a student (except
homeless students as defined in A.R.S. 15-824) in the
District for the first time will be asked to produce one of
the following proofs:

A certified copy of
the child's birth certificate.

Other reliable proof of the student's
identity and age, including the student's baptismal
certificate, an application for a Social Security
number, or original school registration records,
and an affidavit explaining the
inability to provide a copy of the birth certificate.

A letter from the authorized
representative of an agency having custody of the
student (pursuant to statute) certifying that the
student has been placed in the custody of the agency as
prescribed by law.
The parent, guardian, or surrogate will be
given thirty days to provide documentation requested as
listed above. If documentation is not provided, a letter
will be sent to notify the parent, guardian, or surrogate
that unless the documentation is provided within ten days,
the local law enforcement agency will be notified.
Nothing contained in this policy shall
authorize the District to disclose to any person a student's
educational record without prior parental consent unless the
District makes a determination that disclosure of such
records is necessary to protect the health and safety of the
student.
