*Birth certificate
*Immunization records
*Proof of residency
Most common forms for proof of residency are a water or electric bill. 
We also accept lease or mortgage agreements, bank statements, and driver’s licenses as long as they have a valid PHYSICAL address listed.

Voter Registration: 3 ways to get registered
You can go to the Apache County Recorder’s page and print the form.
You can go to Arizona's voter registration site and complete the form online. 
You also can go to the post office and get one there and fill it out and mail it in.

Arizona State Tax Credit for Round Valley Schools.
The money you pay will be reducing your income tax bill dollar for dollar,
and will go directly to programs of your choice.
